We will bill your insurance for services if we are a participating provider for that insurance plan. If we are not a participating provider, you may submit your own claim for reimbursement for the fees you have paid. We can provide assistance in locating forms for reimbursement.

We will attempt to obtain verification of your insurance benefits prior to your visit. Please check with your insurer to understand the details of your policy and ensure that you are aware of co-pays and deductibles. Having insurance does not guarantee that your services will be covered.  

 

Medicare does not cover the refraction portion of the eye exam. If your Medicare supplement does not cover this service you will be responsible for the fee.
