What To Bring

Insurance card and a photo ID.

Be prepared to provide a list of your current medications, health history, allergies.

Bring your glasses. Even if they are outdated or broken, it can be helpful to know what you have been prescribed in the past. If you wear over the counter reading glasses, bring those too.

Have contacts? If you are a contact lens wearer, and your lenses are not causing any discomfort, have them inserted at minimum an hour prior to your visit so they are properly settled on your eyes for evaluation. If you are a new patient and have any identifying information about your contacts such as a copy of the prescription or the packaging it can be helpful in identifying if what you are using is the best fit for you and your needs.  

You may want to bring a driver if you feel uncomfortable driving after a dilated eye exam.
